• To enroll your child in Little Star Daycare, all the enrollment forms must be completed and turned in before your child's first day of care. All the enrollment documents can be found on this page. If you would like, you may print and fill them out at home. Completed forms can be emailed to krystina.littlestar@gmail.com or dropped off at Little Star Daycare located in Bethlehem Lutheran Church on 2245 Hayden Ave. Altoona, WI 54720. Please feel free to come in or call with any questions regarding these documents.
  • Little Star Daycare serves children ages 2-13 years of age. The facility is licensed for 61 children.
  • Little Star Daycare 2 serves children ages 6 weeks - 23 months. The facility is licensed for 12 children.
  • Additional documents will be given to you at your scheduled orientation. These documents will include other helpful information about the child care center, parent handbook, and classroom welcome letters. You may also receive a food program sheet regarding your child's needs/and schedule. This may include informing the center if your child will need to be fed breakfast, snack, or lunch.
